The procedure has an outdated reputation it no longer deserves: modern root canals are comfortable, efficient, and quiet. The pain people associate with root canals is the toothache before treatment. The root canal is what ends that pain, and keeping your natural tooth remains the best outcome in dentistry.
After thorough numbing, we access the inflamed pulp through a small opening, remove it, then clean, disinfect, and shape the canals inside the roots. The canals are sealed with a biocompatible filling material, and the tooth is protected, usually with a crown, so it serves you for years. Precise imaging guides the whole process, and with our same day crown technology, many teeth are fully protected before infection ever gets a second chance.
Root canals relieve severe pain, usually immediately, stop infection from spreading into the jaw and beyond, and preserve your natural tooth, which is almost always better than any replacement. Keeping the tooth maintains your bite, your chewing efficiency, and the bone around the root. The alternative, extraction, ultimately costs more once you replace the tooth properly. If you have lingering pain with hot or cold that lasts more than a few seconds, spontaneous throbbing that wakes you at night, pain when biting down, swelling near a tooth, or a pimple like bump on the gums, the nerve inside a tooth may be inflamed or infected, and a root canal may be exactly what ends it. Deep decay, cracks, and repeated dental work on the same tooth are the usual culprits. A quick exam and image gives us the answer definitively.
Modern root canals feel much like getting a filling, just a bit longer. You are numbed completely before anything begins, and staying comfortable is the priority throughout; nitrous oxide is available if nerves are part of the picture. Most patients are stunned by how uneventful it is, and deeply relieved when pain that had been ruining their sleep is simply gone. The most common thing we hear afterward is that was nothing like I expected.
Most root canals at our Rancho Bernardo office are completed in a single visit of 60 to 90 minutes. Molars with complex canal anatomy or significant infection sometimes need a second visit a week or two later. If the tooth needs a crown afterward, and most back teeth do, our same day crown technology often lets us complete root canal and crown in one extended appointment, one numbing, done. From emergency call to pain relief is usually measured in hours here, not days.
A root canal treated back tooth almost always needs a crown; without one, the tooth can crack and be lost despite a perfect root canal, so do not skip that step. Mild soreness for a few days afterward is normal and different from the original pain. Rarely, a canal can reinfect years later and need retreatment; we are upfront about those odds, which are low. With a crown and normal hygiene, a root canal treated tooth routinely lasts as long as its neighbors.
Expect the deep, throbbing pain to be gone immediately, replaced by mild tenderness when chewing for three to five days as the tissues around the root settle. Ibuprofen handles it well for most patients. Chew on the other side until your permanent crown is placed, and avoid very hard or sticky foods on the tooth in the meantime. Most people return to work the same day or next morning. If swelling or significant pain returns, call us promptly; that is not the normal course.
