They are different procedures for different conditions, one is maintenance, the other is treatment, and confusing them is like confusing an oil change with an engine repair. The decision comes from your measurements, not from anyone's opinion: pocket depths, bleeding points, tartar below the gumline, and bone levels on your imaging. Healthy readings of one to three millimeters mean a prophy maintains what is working. Pockets of four to five millimeters and beyond with bleeding and buildup mean bacteria have colonized below where a prophy reaches, and scaling and root planing is what evicts them and smooths the roots so gums can reattach.
Getting the right cleaning matters in both directions. A prophy performed on diseased pockets polishes the visible while infection advances unseen, undertreatment with a clean conscience. We show you your own measurements and imaging so the recommendation makes sense before you commit to anything, pocket numbers on screen, bleeding points marked, tartar visible on your films. You will never be pushed into treatment you do not need, and you are welcome to take the findings home or seek a second opinion without an ounce of friction. Honest dentistry survives scrutiny comfortably; that is rather the point of it.
The determination happens inside a regular exam: probing takes minutes, imaging is reviewed on the spot, and you get the verdict with evidence the same visit. If a prophy is right, it happens then and there. If SRP is indicated, it is typically scheduled as two visits of 60 to 90 minutes, one side each, with a reevaluation four to six weeks later that measures the improvement in the same numbers that justified treatment.
Two honest flags worth knowing. Be cautious of a deep cleaning recommended without probing numbers and imaging you can see, evidence is not optional. Be equally cautious of your own instinct to decline SRP that is genuinely indicated, because a prophy on diseased gums treats nothing, and pockets do not negotiate. A prophy has no recovery to speak of.
